Job Description
The Johns Hopkins John G. Bartlett Specialty Practice Ryan White Adult HIV/AIDS Program provides comprehensive health and social services to adults living with or affected by HIV/AIDS at the Johns Hopkins Hospital. The mission of the John G. Bartlett Specialty Practice is to provide outstanding, evidence-based, clinical infectious disease care with access for all. The Bartlett Ryan White Adult HIV Program is a fully grant-funded program that fills gaps in services needed for people at high risk for HIV infection and for people living with HIV. Specifically, the program is funded by the competitive Ryan White federal program requiring the development of grant applications responsive to the sponsor as well as reports demonstrating how the program is utilizing funds and is meeting the proposed benchmarks and outcomes to assure continued funding.
We are seeking an HIV/AIDS Program Manager. Under limited supervision and in collaboration with the Director of the Bartlett Ryan White Service and the Director of Bartlett Social Work, the HIV/AIDS Program Manager will oversee and manage programmatic activities of Ryan White sponsored projects in the John G. Bartlett Specialty Practice to ensure delivery of high-quality HIV prevention and treatment and to promote the mission, goals, and the strategic vision of the John G. Bartlett Specialty Practice. Primary responsibilities include the provision of case management services to clients at increased risk for HIV infection and people living with HIV who are newly diagnosed or previously diagnosed with HIV; oversight of Bartlett Ryan White grant reporting and performance measure monitoring; facilitating collaborations with internal and external partners on implementation and development of Bartlett Ryan White programs; developing, implementing, and monitoring policies and procedures for the Bartlett Early Intervention Services program that offers HIV testing, PrEP, HIV linkage, engagement, and retention services, and services offered through the Bartlett Telehealth program; supervision of Bartlett HIV, PrEP, and Telehealth peer navigators.
Specific Duties & Responsibilities
35% - Direct provision of case management services
- Assist patients in applying for health insurance and prescription programs such as ACA, MADAP, Medicare, Medical Assistance, Social Security Extra Help, QMB, and SPDAP.
- Refer patients to the Department of Social Services for programs such as Food Stamps, TDAP, TCA, Eviction Prevention, and Energy Assistance.
- Provide education on the disability programs provided by the Social Security Administration such as SSI and SSDI.
- Refer patients to community resources programs for transportation, housing/shelters, food services, and employment.
- Assist patients with accessing insurance and/or programs to support payment for PrEP and HIV medication and clinical services when needed.
- Assist patients to enroll in transportation programs offered by the MTA and Medical Assistance.
- Educate patients referred for PrEP about PrEP services and facilitate PrEP appointments for those interested in/potentially eligible for PrEP.
- Educate these consumers on the availability and importance of PrEP services and assist the client in finding the best location for care.
- Assist the assigned client(s) in overcoming barriers to accessing PrEP, primary medical care, and support services.
- Educate patients referred for HIV care about HIV services and facilitate HIV clinical appointments for those linking and/or re-engaging in HIV care.
- Educate these consumers on the availability and importance of HIV services.
- Assist the assigned client(s) in overcoming barriers to accessing HIV care and support services.
- Reinforce with clients the need to keep health care appointments.
- Provide inspiration/motivation for persons struggling with medication and/or appointment adherence and/or psychosocial issues (i.e. sexual orientation, gender identity, unemployment, mental health issues, addiction) through support, education, and connection to psychosocial resources as indicated.
- Help patients develop methods for self-management; assist patients in developing strategies to help them adhere to medications and appointments.
- Encourage patients to use supportive services and counseling.
- Educate patients on self-advocacy skills and empower them to communicate effectively with their healthcare team.
- Reinforce HIV prevention and harm reduction information as needed.
- Provide resource materials to clients and agency staff.
- Conduct follow-up telephone calls and other contacts with clients to ensure that services were obtained; reschedule appointments as needed.
- Document and provide pertinent information regarding services delivered.
- Establish and maintain working relations with the clinical care team.
- Maintain client confidentiality.
- Maintain professional relationships with clients.
25% - Adult Ryan White Program Oversight & collaboration with External Agencies
- Work closely with the Director of Adult Ryan White Services, and Bartlett Social Work Director to complete grants, and Ryan White grants quarterly narrative reports.
- Provide administrative oversight for HIV supportive programs: a. monitor funds allocated for support groups, assist with the development and implementation of support group work plans, c. monitor data collection protocols, data entry, and quarterly reports. Assist with processing of receipts and reconciling grant expenditures.
- Establish and maintain relationships with community partners by attending meetings, town halls, and events.
- Provide leadership and coordination of monthly meetings with partnering community-based organizations.
- Monitor progress towards meeting program goals for internal and external collaborations.
- Work closely with program managers and directors for Infectious Diseases on developing and monitoring community engagement and health promotion initiatives for the Bartlett Clinic.
- Promote knowledge of and access to Bartlett Ryan White program resources and activities with Bartlett providers, staff, and patients.
- Represent the program in correspondence and through meeting attendance with funding agencies, including the Baltimore City Health Department, the Maryland Department of Health and Mental Hygiene, the US Health Resources and Services Administration, and local, regional, and national public health and educational meetings and conferences.
- Collaborates with the Bartlett Ryan White Program Director and Bartlett Director of Social Work to provide Ryan White program leadership and representation in Bartlett Clinic. Management meetings and other interdisciplinary clinical teams involving HIV-related programs in the Johns Hopkins Hospital.
- Collaborate with Bartlett navigators to conduct HIV prevention and treatment outreach and community events.
25% - Bartlett Ryan White EIS and Telehealth Program Oversight and Implementation
- Lead with the oversight of the day-to-day operations for the Early Intervention Services and Bartlett Telehealth Services Program.
- Work closely with the Ryan White Director and Director of Social Work to complete grants and required narrative reports for funds that support the Bartlett HIV EIS and Telehealth Programs.
- Liaise with the Grants Team on required grants Accountable for grant submission administration, and regulatory compliance.
- Be responsible for data collection and reporting of the Early Intervention Services and Telehealth Program activities.
- Work with the Ryan White Director, project leads, and patient/peer navigators to develop and implement: data collection protocols, internal and external PrEP and HIV treatment referral protocols for high-risk HIV-negative individuals and patients linking and re-engaging in HIV care, respectively; b. entry and retention protocols for Bartlett clinic PrEP and HIV-positive patients; and c. protocols to expand the availability of PrEP and Rapid Start case management and patient navigation services to other Johns Hopkins primary care sites that can provide PrEP and Rapid Start services.
- Establish program goals and objectives with the Early Intervention Services and Telehealth team and monitor progress.
- Manage the Early Intervention Services and Telehealth program budget for patient education materials, office supplies, and other program functions.
- Develop and enforce HIPPA Compliant telehealth workflows when delivering HIV-supportive services virtually.
- Develop HIV Prevention, HIV Treatment, and Telehealth marketing materials.
15% - Supervision
- Supervise Bartlett PrEP, HIV, and Telehealth navigators.
- Provide leadership and coordination of monthly meetings for Bartlett Ryan PrEP, HIV, and Telehealth navigators.
- Implement and monitor individual development plans for Bartlett Ryan PrEP, HIV, and Telehealth navigators.
- Supervise and train the Bartlett Ryan PrEP, HIV, and Telehealth navigators to provide patient wellness and telemedicine outreach services.
Minimum Qualifications
- Bachelor’s Degree in Social Work, Psychology, or another related field.
- Requires a minimum of five years of related experience, preferably in a healthcare or public welfare setting with knowledge of HIV prevention, substance abuse, and psychiatric illness.
- Additional education may substitute for some experience, to the extent permitted by the JHU equivalency formula.
Preferred Qualifications
- Demonstrate general knowledge of HIV prevention, HIV treatment, lesbian/gay/gender identity issues, and resources.
- Demonstrate knowledge specific to public entitlement programs related to health care.
- Able to work with culturally diverse patients, staff, and community populations.
- Demonstrate strong organizational skills with attention to detail and follow-through.
- Experience supervising others and in program management preferred.
