General Statement of Job:

The Care Manager Extender will report to the Tailored Care Manager and with providing whole person care to consumers. Care Manager Extenders will perform functions within their training, content, and abilities. The Care Manager Extender is in regular communication with supervising Care Managers and engage in coordinating efforts to ensure consumer enrollment and engagement in Tailored Care Management Services through direct care/contact.

 

Qualification and Education Requirements:

It is required that the applicant of the role must be at least 18 years of age and must have obtained at minimum a High School Diploma /GED. Applicants of the role must also meet at least one of the below requirements.

Be a person with lived experience in working with individuals with Mental Health, Substance Use and/or I/DD diagnosis. Must have demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system.

Must have at least two years of direct experience providing services for and navigating the Medicaid delivery system on behalf of that individual.

Has two years of paid experience performing the types of functions described in the Duties / Responsibilities section of this role description in addition to one year of paid experience working directly with the Tailored Care Management eligible population.

Essential job Duties or Responsibilities

· The Care Manager Extender will provide integrated care management in service sites, homes, and communities through face-to-face and other interactions and the work of integrated care teams.

· Engage with the team in the development of care management plans for consumers

· Referring and linking to needed services, monitoring / follow up with client and referrals, provide education for health promotion. Ensure metrics for outcomes are met.

· Participates in interdisciplinary treatment planning, consultation activities and ensures all involved parties are aware of the plan of care.

· Provides crisis intervention consultation to all assigned participants of TCM.

· Provides Outreach and Engagement

· Engage consumers in health promotion activities.

· Effectively implement organizational priorities, quality initiatives and programs

· Represent Selfless Foundation at designated community stakeholder, provider and NC Department of Health and Human Services related meetings.

Paperwork / documentation to ensure that activities are carried out within the assigned area in an effective manner.

· Provide each consumer with the minimum amount of monthly required contact hours.

· Participate in and maintain Care Management and Selfless Foundation trainings and proficiencies.

· Performing general outreach, engagement, and follow-up with members

· Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation)

· Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing

· Sharing information with the care manager and other members of the care team on the member’s circumstances

· Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services

· Participating in case conferences

· Support the care manager in assessing and addressing unmet health-related resource needs.

 

Care manager/extender contacts for members with behavioral health needs:

 

High Acuity: At least four care manager/extender-to-member contacts per month, including at least one in-person contact with the member.

 

Moderate Acuity: At least three care manager/extender-to-member contacts per month and at least one in-person contact with the member quarterly (includes care management comprehensive assessment if it was conducted in- person).

 

Low Acuity: At least two care manager/extender-to-member contacts per month and at least two in-person contacts with the member per year, approximately six months apart (includes the care management comprehensive assessment if it was conducted in-person).

 

Tailored Care Management Training includes

· Tailored Plan eligibility and services

· Person health and unmet resource needs

· Components of Health Home / Tailored Care management

· Ethics, boundaries, and personal safety

· Building a trusting relationship

· Community integration

· Health promotion

· MH/SU and /or I/DD specific trainings

· Transitional Care Management Service Definition and Delivery

· Skills to support health behavior change, including motivational interviewing

· Person-centered needs assessment and care planning

· Preparing members for and assisting them during emergencies and natural disasters

· Infection control and prevention practices

· Understanding virtual / telehealth applications

· Needs of justice-involved individuals

· Additional trainings for servicng

· Children with complex needs

· Pregnant and postpartum persons with substance use disorder (SUD) or with SUD history

· Members with LTSS need.

· Must complete all TCM trainings within 6 months of hire date.

 

Skills and Abilities:

· Ability to establish appropriate and respectful relationships / partnerships with person with a wide range of disabilities and healthcare conditions and their families

· Ability to interact professionally and effectively with persons who are upset and who disagree

· Ability to express ideas clearly and concisely orally and in writing

· Ability to work as a team member and under the direction of the assigned Tailor Care Manager.

 

Residency Requirement:

In accordance with the MH/SU/BH and I/DD Tailored Plan requirements mandated by the North Carolina Department of Health and Human Services, Selfless Foundation health positions are required to be filled by individuals who reside in North Carolina, meaning someone who establishes a legal domicile in North Carolina and pays income tax in North Carolina, or resides within 40 miles of the North Carolina border. New hires from outside of North Carolina will have 60 days from the date of hire to meet this requirement, if applicable to the position.

Location: Hybrid /Remote position to serve consumers for the counties of Forsyth, Yadkin, Davie, Rockingham, Guilford, Randolph and/or Davidson

Status: Full-Time

Salary: $22-$24 per hour

Effective: 

Job Type: Full-time

Pay: $22.00 - $24.00 per hour

Expected hours: 35 per week
