Your Role
The Clinical Access Program's Initial Health Appointment Outreach Team ensures that all newly enrolled Medi-Cal members have an IHA visit with their PCP within 120 days of enrollment. The Clinical Services Coordinator will report to the Clinical Training and Auditing Supervisor. In this role you will be responsible for making outreach calls to Medi-Cal members on a daily basis and maintaining documentation of this outreach
Your Knowledge and Experience
- A minimum of a high school diploma
- Requires at least 3 years of prior relevant experience
- Bilingual Spanish Prefered
- Expertise in Microsoft Office (Word, Excel, Outlook), Sales Force (Care Connect), FACETS
- Capable of working independently and as part of a team
- Excellent written and verbal communication skills
- Health Plan, IPA, Public Health, Quality Improvement experience desired
Pay
The pay range for this role is: $ 20.70 to $ 28.98 for California.
