Family Health Centers of San Diego (FHCSD) is looking for a Substance Use Disorder Counselor/Case Manager. SUDC: Provide substance abuse screening, brief interventions, therapy for individuals and groups, and coordinates referrals for patients. Also provide leadership and training to staff related to chemical dependency issues, and serving as a subject matter expert in the cross-functional treatment team. Case Manager: Provide intensive, therapeutic case management services to assigned individuals. Participate and supports, the planning, development, implementation, and evaluation of services in accordance with contractual and departmental requirements and guidelines. Specific assignments vary depending on program, grant, and patient demographics, with a focus on providing community-based healthcare services to the medically underserved. You will offer intensive case management services to clients and conducts therapeutic interventions.

FHCSD Offers You:

  • Sign-on bonus of $2,000, to be paid after 6-months of service
  • Retention bonus up to 15% of annual pay to a max of $10,000, to be paid after one year of services
  • Loan repayment & scholarships up to $25,000 - available for a limited time through partnership with HCAi (Ca Dept of Health Care Access and Information)

Responsibilities:

  • Conducts initial and on-going assessment of client's health and/or support service needs. Sets level of client need.
  • Creates formalized case plans and goals with clients; clients are high-need and require more intense services. Provides substance abuse screening and assessments. Participates in the development of patients' individualized care plans. Assesses patients' need for enrollment in medical, self-help programs, outpatient services, and/or residential facilities and makes referrals, if necessary. Updates plan as client's needs change.
  • Provides therapeutic interventions and intensive individual support to clients. Support may include providing therapeutic interventions, providing internal and community services referrals, and more intensive support may include accompanying clients to housing services, appointments, social services, etc. Focus on medical adherence, support, and appointments designed to maintaining necessary care.
  • Provides substance abuse education and referral information to patients, families, and other staff members in the areas of substance abuse counseling and treatment services.
  • Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
  • Develops and maintains community resources information and contacts. Acts as a liaison and may provide consultations with community agencies.
  • Participates in the design and implementation of mental health and substance abuse service delivery, working closely with multi-disciplinary team to ensure continuity of care.
  • Provides direct substance abuse counseling services, including crisis intervention and individual, group, and family counseling, targeting patient population groups experiencing substance abuse/dependency disorders.
  • Performs other duties as assigned.

Requirements:

  • Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field required. Master's degree in relevant field preferred.
  • High School diploma (or GED equivalent) required.
  • Registered Intern status or certification from a California State Substance Use Disorders (SUD) Services certifying agency required.
  • 1 year experience in providing counseling required; 2 years of experience providing counseling to assigned patient demographics, such as patients diagnosed with serious mental illness or HIV, preferred.
  • 3 years of work experience providing human services to high risk, medically underserved, or relevant community health populations required.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

$27.50 - $33.50

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
